Bacteriological Study for Drains after Biliary Surgery

Abstract
The need to drain the peritoneal cavity following abdominal surgery has long been a subject of controversy. Many authors believe that the peritoneum deals competently with fluid and infection, but other authors believe that drainage can limit postoperative complications particularly in the presence of peritonitis.
But drains are used still for one of two: either to prevent accumulation of fluid or secretions in a operative wound, or to permit escape of fluid, pus or blood which has already accumulation.
In this paper, 107 cases of biliary surgery were examined with the direct smear and bacterial cultures of the initial 1em of the drain, which were inserted into foraman of Winslow or Morison¢¥s pouch and removed on the postoperative fifth day. And we also studied the efficacy of the topical antibiotics sprays on the operative fields.
The results are as follows.
1) The-cases with bacteriological complications are not many (18.7%) as the opponents emphasize.
2) Moreover only two cases of total 20 cases of positive culture showed normal skin flora.
3) The positive cultures are related to the long and difficult management in operation.
4) Therefore, the preventive drain insertion after biliary surgery are needed for drainage of bile leakage from the aberrant ducts or liver bed.
